What will I need to bring when I register my new Maxis mobile corporate account?
Here is a checklist of documents you will need to bring to a Maxis Centre or authorised dealer when you want to register your new Maxis mobile account.

1. A copy of Form 9 or 13 and Form 49 for Sendirian Berhad.
2. A copy of Form 8 or 13 and Form 49 for Berhad.
3. A new format Identity Card or MyKad issued at age 18 or a valid passport. If you submit a temporary I.C., an old format I.C. or a new format I.C. issued at age 12, you must attach either one of the supporting documents listed below.
4. Letter of authorisation by an authorised signatory, stating name, position and IC number.
5. A fixed-line phone number (not mobile phone number) must be included with each application.
6. Company stamp.

Supporting documents for individuals:

• A copy of valid driving licence
  
• Valid passport

• Letter of confirmation from your employer with the following details:
• Current date
  
• Your full name
  
• Your I.C. number
  
• Your designation
  
• The duration of your employment
  
  
• Or, a copy of a recent and computerised payslip
  
• Or, a copy of the latest Form J
  
• Or, a copy of a recent bank statement or credit card statement
